Identity of the data controller
Affidea is a healthcare provider with high ethical standards. "Affidea" refers to the Affidea group, under the entity AFFIDEA ESPÑA QUALITY, SLU and/or any other Affidea subsidiary entity in Spain. Your contact is our data protection officer in Spain. Contact details can be found in clause 6.
Data processing purposes
Affidea processes your personal data contained in the patient's referral to us. In the event that we perform a medical service to a patient you refer to us, the referral becomes part of our medical record.
Affidea processes your personal data to facilitate the provision of Affidea's healthcare services. We are committed to serving our patients in the most efficient manner. To this end we will contact you directly by email or telephone when your information is needed. Typically in connection with our healthcare services it may be necessary to contact you for an inquiry about a referral you have issued or a request for clinical information about the patient (e.g., medical results).
Affidea processes your personal data for the satisfaction survey. We would like to understand your opinion about the quality of our services, so that we can improve our operations and serve patients in the best possible way.
Affidea processes your personal data for statistical analysis of aggregated data. We want to analyze prescriptions on an aggregated basis to obtain valuable statistical information for our operations and commercial teams, for example, to understand which services are of most interest in specific regions. If you prefer, we will exclude your data from our analysis.
Affidea processes your personal data also for promotional information and other events. We would like to make you aware of our medical services and keep you up to date with changes or new services. Unless you mark your non-compliance, we will contact you via email to provide you with relevant information about Affidea's current medical services and inform you about our events, conferences and seminars.

With whom we share your personal data
Affidea uses service providers (called processors) to help us process the personal data we collect. The processors act on behalf of Affidea and based on our written instructions.

Your data protection rights
Under data protection legislation you have the following rights.
1) Right to request access to your personal data. This means that you have the right to obtain a confirmation that data is being processed. You have access to your personal data processed by us and other supplementary information (especially the purposes of the processing, the categories of personal data concerned, the recipients or categories of recipients to whom the personal data have been or will be disclosed and the retention period).
2) The right to request the rectification of your data. This means that you have the right to correct your personal data by completing it if it is incorrect or incomplete.
3) The right to request the deletion of your data. This means that you have the right to have your personal data deleted in specific circumstances, if there is no reason to continue our lawful processing.
4) The right to request that we restrict processing. This means that you can request to 'block' or delete processing of your personal data in specific circumstances. Your valid request results in us being allowed to store your personal data, but not to process it.
5) Right to object to the processing of your data. This means that you have the right to refuse our activity to send you survey and/or promotional information. Your right to object also means that you have the right to challenge the data processing Affidea carries out on grounds relating to your situation, if the lawful basis for the data processing is our legitimate interest.
If you wish to exercise your rights or would like to have more information, please contact our Data Protection Officer. If you are dissatisfied with the way we process your data, you have the right to lodge a complaint to the Supervisory Authority, Spanish Data Protection Agency. Or to initiate proceedings in court, however, please raise any issues with us in the first instance.
